• Clinical Appraisal Form - this form must be completed for clarification of clinical skills and can be utilised by both member and client. Once completed can be emailed to admin@ausnursingnetwork.com.au or faxed to 1300 653 741
• QSIR Form - should you identify a hazard in your workplace or an accident or incident occurs in your workplace please complete this form and fax to 1300 653 741 or emailed to admin@ausnursingnetwork.com.au immediately. All hazards, incidents or accidents at work must be reported to your employer immediately.
• Change of Details form - use this form if you need to change any personal details such as banking details, address, contact details etc. Once completed you can email to admin@ausnursingnetwork.com.au or fax to 1300 653 741
• Host Employer Induction Checklist - As part of our employer responsibilities all agency staff must complete an induction checklist for each health care facility they work at. Once you complete your induction form fax to 1300 653 741
• Super Choice Form - As an ATO requirement all members of Australian Nursing Network must complete a super choice form. Once completed please fax to 1300 653 741
